In order to have your children participate with Parks and Recreation's Little League, guardians will have to bring and fill forms for the safety of the children. The following items will be required, all attached below as PDF documents. Tri-Forms can be picked up at the front desk of our facility. For any other questions, please contact us.
1. Original Birth Certificate for all ages. Abide my Little League Age Chart

Softball Age Chart Baseball Age Chart
2. Registration Form
3. Medical Release Form
4. Parent Code of Conduct
*FORMS NEEDED FOR ALL-STARS - Ages 9-14 only*
1. School Enrollment Form
and
2. 3 Proofs of Residency
- For ALL-STARS ages 9-14 only.
- Must Provide One or more documents from EACH of the following Groups:
